Northwestern Medicine is seeking a Physical Therapist IHR to join our team at Lake Forest Outpatient. This is a casual, out-of-hours position that requires a minimum of 16 hours per month.
Key Responsibilities- Develop and implement physical therapy programs to maximize patient function and educate patients on their limitations.
- Evaluate patients and gather information to determine accurate clinical diagnoses and treatment plans.
- Communicate treatment programs to patients, families, and interdisciplinary personnel, providing emotional support as needed.
- Coordinate and implement treatment plans, modifying as necessary based on patient progression.
- Treat patients using various techniques, including therapeutic exercise, functional activities, manual techniques, and modalities.
- Evaluate home, work, and community settings and make recommendations for safe and efficient function.
- Re-evaluate patients at appropriate timeframes and document goal progression to maintain effective plans of care.
- Plan and communicate discharge to patients, families, and medical personnel in a timely fashion.
- Maintain accurate and thorough documentation in accordance with department policy.
- Bachelor of Science in Physical Therapy
- Current State of Illinois Physical Therapy license or in process of obtaining
- BLS through the American Heart Association
