This includes assessing patients' physical strengths and limitations, developing and implementing treatment plans, and collaborating with other healthcare professionals to ensure comprehensive care.

Responsibilities:
  • Plan and administer physical therapy treatment programs for patients in their homes
  • Assess patients' physical strengths and limitations to develop and implement effective treatment plans
  • Collaborate with other healthcare professionals to ensure comprehensive care
  • Document patients' progress and treatment plans accurately and thoroughly
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care
Requirements:
  • Doctor of Physical Therapy (DPT) degree from an institution accredited by the American Physical Therapy Association (APTA)
  • Current and unrestricted license to practice Physical Therapy in the State of California
  • Current CPR/BLS certification
  • Compliant with state healthcare regulatory requirements
  • Ability to care for patients with different personalities and needs
  • Ability to assess patient condition and work with treatment plans
  • Minimum of 1 year of recent Physical Therapy experience in an institutional setting
  • Minimum of 1 year of recent Physical Therapy experience with a home health agency
