Job Summary

Key Responsibilities
  • Develop and maintain relationships with residents and their families, prioritizing their physical, spiritual, and emotional needs.
  • Assess and evaluate resident clinical conditions, documenting findings and notifying providers as necessary.
  • Update and modify care plans to reflect changes in resident care and service needs.
  • Supervise and lead care associates, ensuring excellent care delivery.
  • Administer medications accurately and timely, following orders and protocols.
  • Participate in meetings related to resident outcomes and care, contributing to continuous improvement.
  • Respond to resident, family, or responsible party inquiries in a timely and professional manner.
  • Participate in clinical indicator reviews, focusing on falls, wounds, nutrition, and antipsychotic use.

Requirements
  • Graduate of an approved Licensed Practical/Vocational Nursing program.
  • Minimum 1 year of experience in assisted living, home health, or long-term care.
  • Passion for caring for seniors and advocating for their needs.
  • Experience with electronic medical records preferred.
Compensation

LPNs: $29-$37 per hour, depending on experience.

RNs: $37-$49 per hour, depending on experience.

Shift differential: $2/hour.
