Reliant - Chehalem Health and Rehab Center - NSS is seeking an experienced Occupational Therapist to join their team in a Skilled Nursing Facility setting.
The ideal candidate will have a strong background in managing treatment plans to promote positive outcomes for patients with diverse diagnoses.
Key responsibilities include developing and implementing individualized treatment plans, conducting assessments, and collaborating with interdisciplinary teams to ensure comprehensive care.
A minimum of one year of recent experience in a Skilled Nursing Facility is preferred.
