Illumia Health, a leading provider of hospice and palliative care, seeks a skilled Registered Nurse to join our team as an Advanced Illness Management RN. This role requires providing critical support in managing the care of patients with advanced illnesses after hours.

The ideal candidate will have 2 years of prior skilled Registered Nurse experience, with demonstrated ability to provide quality patient care. Nursing experience in palliative care, oncology, hospice, or geriatric care is preferred.

Responsibilities include:

  • Triage patient needs over the phone until a nurse can provide an in-person visit
  • Respond appropriately and with urgency to crisis calls from patients and families after hours
  • Make the initial nursing evaluation in determining eligibility for palliative serves, as part of a patient visit, within 48 hours of referral

The salary range for this position is $70,000 - $90,000 per year, depending on experience and qualifications.
