Job SummaryWe are seeking a skilled and dedicated Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our team. As a Home Health Nurse, you will work with a sweet, 22-year-old man who enjoys the library, being outdoors, books, and balloons. Our patient has generalized developmental delays and requires assistance with seizure activity, oxygen titration, ambulation, pain management, help with eating and drinking, and monitoring for aspiration.
Responsibilities- Provide high-quality, patient-centered care to our patients and their families.
- Assess and develop individualized care plans to meet the unique needs of each patient.
- Implement and evaluate the effectiveness of care plans, making adjustments as needed.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Participate in ongoing education and training to stay current with best practices and industry developments.
- Currently licensed and registered to practice as a Registered Nurse/LPN with the State Board of Nursing in Colorado or other Nurse Compact State.
- Valid driver's license and access to a vehicle that is insured and in good working order.
- Ability to travel within assigned geographic areas.
- Current CPR for Healthcare Providers.
