MDS Coordinator

We are seeking a highly skilled and experienced MDS Coordinator to join our team at Navasota Nursing & Rehabilitation. As a key member of our clinical staff, you will play a critical role in ensuring the highest quality of care for our residents.

• Schedule residents for assessments and ensure timely completion of the comprehensive assessment (MDS)
• Monitor and maintain accurate and up-to-date documentation of resident care plans
• Coordinate and attend regular Medicaid meetings to discuss resident care and updates
• Perform care plan updates and provide reports to the DON and Administrator
• Remain current with industry changes and best practices through ongoing education and training

• Licensed Nurse in good standing
• Geriatric nursing background and experience
• Excellent communication, organizational, and time management skills
• Ability to work independently and as part of a team
