Job Title: LPN - InVision Human Services

This position will be responsible for coordinating the medical care and treatment of individuals supported by InVision Human Services.

Key Responsibilities:
  • Review and maintain medical history and records, ensuring compliance with regulations and best practices.
  • Ensure physician-recommended treatment is received in accordance with regulations and best practices.
  • Serve as an intermediary between InVision internal teams and community providers, including primary care providers, specialty providers, and regulatory agencies.
  • Provide education and training to staff, individuals supported, and their designees on medical diagnoses, prescribed medication, and mobility or positioning.
  • Develop and implement standardized and individualized protocols for diagnoses affecting the IDD population.
  • Observe, evaluate, and prompt medical intervention as needed.
  • Participate in team meetings and health-related goal development, implementation, and outcome reporting.
  • Perform basic assessments of physical and mental health needs and obtain vital signs as needed.
  • Accompany individuals supported and their staff to medical appointments as needed.
  • Support residential teams with compliance standards set forth in the 6100 and 6400 regulations.
  • Coordinate with external medical professionals, community providers, and regulatory agencies as needed.
  • Advocate for the needs and rights of individuals supported when hospitalized or in treatment.
  • Serve as an on-call nurse as needed.
Requirements:
  • Current PA Licensed Practical Nurse.
  • Professional experience with the intellectually/developmentally disabled population, a plus.
  • Professional experience with the medically and behaviorally complex, a plus.
  • Experience with Electronic Health Records (EHR).
  • Valid PA Driver's License.
  • Excellent written and verbal communication skills.
  • Ability to maintain confidential information within HIPAA guidelines and organizational policies.
Physical Requirements:

While performing the duties of this job, the employee is regularly required to sit, use hands to manipulate objects, tools or controls, reach with hands and arms, and talk and hear.

The employee must frequently lift and/or move up to 10 pounds and occasionally lift and/or move up to 50 pounds.

Specific vision abilities required by this job include close vision, distance vision, peripheral vision, depth perception, and the ability to adjust focus.
