UTMB Health is seeking a highly skilled Clinical Pharmacist to join our team. As a key member of our healthcare team, you will be responsible for providing safe, effective, and timely medication therapy to our patients.
Key Responsibilities- Medication Management: Interpret orders for medications, ensure therapy is appropriate for patient's diagnosis, weight, concomitant medications, drug allergies, and age.
- Compounding and Dispensing: Compound and dispense prescribed medications per orders in all patient care areas in accordance with departmental and applicable laws.
- Adverse Drug Event Detection: Detect, prevent, evaluate, and report, when necessary, Adverse Drug Events.
- Medication Counseling: Provide medication counseling to patients to ensure accurate understanding of treatment purpose, duration, special instructions of use, storage, and common adverse effects.
- Quality Assurance: Participate in quality assurance programs in the Department of Pharmacy.
- Supervision and Training: Supervise the activities of supportive personnel and reinforce policies and procedures. Assist in the training and development of departmental staff.
- Education: Bachelor of Science in Pharmacy or Doctor of Pharmacy degree.
- Licensure: Licensed to practice pharmacy in the state of Texas or eligible to practice pharmacy in the state of Texas.
- Immunization Certification: Obtain Immunization Certification within six (6) months of hire.
- Experience: Three (3) years of related experience.
- Residency Training: PGY1 or PGY2 Residency training.
