LPN Supervisor

Concordia Visiting Nurses is a Medicare-certified, state-licensed, and Medicaid-approved home health agency serving nine Western Pennsylvania counties. We have been providing high-quality skilled nursing, palliative care, and therapy services to our patients for over a decade.

Job Summary:The LPN Supervisor is a licensed healthcare professional responsible for ensuring patient care is coordinated and managed according to state and federal guidelines. This role assists the interdisciplinary team in planning, implementing, and evaluating patient care plans to ensure high-quality healthcare delivery.
Responsibilities:- Reviews clinical documentation for compliance and completeness in a timely manner.
- Notifies physicians of negative trending vital signs.
- Oversees and supervises team members to ensure quality care is provided.
- Evaluates patient conditions for additional services, equipment, and consultations.
- Holds regular meetings to educate team members and facilitate interdisciplinary communication.
- Collaborates with primary clinicians to provide necessary care for positive medical outcomes.
- Plans discharge from service at admission.
- Liaisons between patients, families, physicians, and healthcare providers.
- Provides in-home nursing visits as needed.
- Participates in administrative on-call rotations.
- Reviews Telehealth data and identifies negative results.
- Collaborates with patients, clinical staff, and physicians on negative results or trending.
- Licensed Practical Nurse with a current, valid Pennsylvania license.
- OASIS Certification preferred.
- Two years of progressive experience in home health care.
- Interpersonal, communication, and organizational skills.
- Proficient in electronic health records.
- Knowledge of agency policies, federal and state regulations, and third-party payers.
