Job Summary

We're seeking a skilled Nurse Practitioner to join our team as a Geriatric Care Specialist. In this role, you'll provide primary care to patients in their homes, working collaboratively with our interdisciplinary team to deliver high-quality, patient-centered care. You'll be responsible for conducting initial patient screenings, creating assessment plans, and addressing urgent care needs. You'll also work closely with our community team to complete care assessments and develop personalized care plans.

Responsibilities
  • Provide primary care to patients in their homes, including conducting initial screenings, creating assessment plans, and addressing urgent care needs
  • Work collaboratively with our interdisciplinary team to deliver high-quality, patient-centered care
  • Conduct care assessments and develop personalized care plans in collaboration with our community team
  • Prescribe and manage medications and treatments as appropriate
  • Update patient records and ensure accurate documentation
  • Participate in regularly scheduled team meetings and contribute to the development of care plans
