Job SummaryWe're seeking a skilled Nurse Practitioner to join our team as a Geriatric Care Specialist. In this role, you'll provide primary care to patients in their homes, working collaboratively with our interdisciplinary team to deliver high-quality, patient-centered care. You'll be responsible for conducting initial patient screenings, creating assessment plans, and addressing urgent care needs. You'll also work closely with our community team to complete care assessments and develop personalized care plans.
