Job Summary

We are seeking a skilled Physical Therapist to join our team at Commonwealth Home Health of Moses Taylor. As a Physical Therapist, you will be responsible for assessing and evaluating patient care needs, developing treatment plans, and providing interventions to improve patient well-being.

Responsibilities
  • Evaluate patient's functional status and physical therapy needs
  • Consult with physician to develop therapy plan of care
  • Provide services within scope of practice as defined by state laws
  • Observe and report patient's response to treatment and any changes in condition
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning
  • Instruct and inform participating members of healthcare team, patient, and family/caregivers regarding plan of care, functional limitations, and progress toward goals
  • Visit patients within entire service area of respective agency and other nearby LHC agencies as needed
  • Complete progress notes and submit per LHC policy
  • Develop PT plan of care within state specific guidelines and report outcomes to nurse supervisor within 24 hours of evaluation
  • Participate in staff conferences and committees as requested by agency
  • May supervise home health aides per LHC policy if allowed per state regulation
  • Provide in-service education as requested by agency leadership
  • Adhere to LHC Therapy Standards of Practice in provision of services to all patients
  • Access education to maintain and enhance clinical knowledge and skills that are up to date with current standards of care for profession and to maintain state issued licensure
Requirements
  • Current Physical Therapy licensure in state of practice
  • Current CPR certification
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle
