Responsibilities- Conduct psycho-social evaluations to identify patient needs and develop effective care plans.
- Collaborate with healthcare teams to provide comprehensive care and support to patients and their families.
- Screen high-risk patients, including teenage mothers and those at risk of suicidal or homicidal behavior, for discharge planning and referrals.
- Assist patients and families in accessing appropriate services and resources.
To be successful in this role, you will need:
- A Master's Degree in Social Work or a related field.
- A valid LCSW license.
- At least one year of experience in an acute care setting.
