We are seeking a talented Healthcare Professional Sought for Primary Care Position to join our team. As a primary care professional, you will be responsible for providing high-quality medical care to patients with a wide range of physical, behavioral, and social conditions.
Responsibilities:
- Determine and formulate health histories and perform physical exams.
- Order, interpret, and evaluate diagnostic tests to identify and assess patients' clinical problems.
- Develop and implement person-centered care plans based on available clinical data.
- Conduct ongoing healthcare and clinical management of stable, chronically ill patients.
Requirements:
- Graduate of an accredited Nurse Practitioner Program or equivalent experience.
- Licensure to practice medicine in Oregon or eligibility for such licensure.
- Board certification with NCCPA or equivalent.
