Advanced Primary Care Nurse Practitioner Opportunity in Geriatric Setting

We are seeking an experienced and skilled Primary Care Nurse Practitioner to join our team at LifeSource, Inc. This is a unique opportunity to work with a passionate organization that prioritizes quality care and provides a supportive environment for our clinicians to focus on patient care.
Job Description:
- Conduct comprehensive primary care evaluations to determine individual patient needs, working closely with the Medical Director and other medical providers as necessary.
- Administer medication reviews, documenting findings and providing written documentation for inclusion in patient records.
- Provide services in accordance with LifeSource site-specific protocols, adhering to established standards and policies.
- Upload documentation and billing records daily to the LifeSource EMR system, completing end-of-day tasks outlined in the Clinical Services Manual.
- Maintain open and positive communications with contract site staff, physicians, and interested parties, providing verbal and/or written instruction or feedback regarding medications and other pertinent caregiver information.
- Stay knowledgeable of and adhere to LifeSource standards, policies, and procedures, as well as legal and regulatory agencies' rules and guidelines, and professional ethical standards.
- Exemplify professionalism in all interactions and conduct.
- Perform other duties and activities as assigned by LifeSource Management, maintaining ongoing training to support certifications and licensures.
- Prioritize patient confidentiality and maintain a safe work environment.
- Primary care evaluations: Determine individual patient needs through comprehensive assessments.
- Medication reviews: Administer reviews, document findings, and provide written documentation.
- Service provision: Adhere to LifeSource protocols, standards, and policies.
- Documentation and billing: Upload records daily, complete end-of-day tasks.
- Communication: Maintain open relationships with staff, physicians, and patients.
- Knowledge and adherence: Stay updated on LifeSource policies, regulations, and ethics.
- Professionalism: Demonstrate excellence in interactions and conduct.
- Ongoing training: Support certifications and licensures.
- Confidentiality and safety: Prioritize patient confidentiality and maintain a safe work environment.
To be successful in this role, you will require:
- A Master's degree in Nursing/Physician Assistant Studies from an accredited institution.
- A current and valid license to practice in South Carolina.
- An active Federal DEA number or ability to process one.
- Knowledge of nursing theory, practices, and regulations.
- Awareness of health care regulations and organizational policies.
- Familiarity with common safety hazards and precautions.
- Ability to gather and analyze patient data.
- Skill in evaluating patient problems and providing advice or referrals.
- Proficiency in documenting clinical services.
- High initiative, judgment, and discretion.
- Effective communication and relationship-building skills.
- Calm and composed reaction in emergency situations.
- Interpretation and application of guidelines and procedures.
- Collaboration with healthcare teams.
- Responsible decision-making within scope.
- Evaluation and recommendation for continuous quality improvement.
- Handling confidential and sensitive information.
This role requires walking, sitting, and standing in a normal office environment, lifting and carrying. It also demands working under stress.
