We are seeking a skilled Nurse Practitioner to join our team as a PRN/Part Time Home Risk Assessment provider. As a member of our team, you will have the opportunity to work with a diverse range of patients and develop your skills in home-based care management.

About the Position:

The Nurse Practitioner will conduct comprehensive health assessments in patients' homes, identifying potential health risks and developing personalized care plans to address these risks. You will work collaboratively with our team of clinicians and healthcare professionals to ensure that patients receive the highest quality care and support.

As a PRN/Part Time provider, you will have the flexibility to choose your own schedule and work at times that suit you. You will also have access to our comprehensive benefits package, including mileage reimbursement and opportunities for professional development.

Responsibilities:

The Nurse Practitioner will be responsible for:

  • Conducting comprehensive health assessments in patients' homes
  • Identifying potential health risks and developing personalized care plans
  • Working collaboratively with our team of clinicians and healthcare professionals
  • Providing education and support to patients and their families
  • Documenting patient information and care plans in our electronic health record system

Qualifications:

The ideal candidate will have:

  • A Master's degree in Nursing or a related field
  • Current RN and NP licensure in the state of practice
  • Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty
  • A minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health record systems
  • Excellent verbal and written communication skills

Travel Requirements:

The Nurse Practitioner will be required to travel to patients' homes and other locations as needed. A valid state driver's license and proof of adequate automobile insurance coverage are required.
