Job Title: Field Based Nurse Practitioner

We are seeking a highly motivated and experienced Field Based Nurse Practitioner to join our team at Cigna Healthcare. As a Field Based Nurse Practitioner, you will be responsible for providing high-quality, patient-centered care to individuals with complex and chronic illnesses in the comfort of their own homes.

Responsibilities:
  • Perform home visits with patients to assess their medical needs and develop personalized care plans.
  • Conduct comprehensive evaluations, including physical exams, laboratory tests, and diagnostic imaging, to identify and manage chronic diseases.
  • Develop and implement treatment plans to improve patient outcomes and quality of life.
  • Collaborate with interdisciplinary teams, including social workers, pharmacists, and physicians, to ensure comprehensive care.
  • Provide education and support to patients and their families on chronic disease management, medication adherence, and healthy lifestyle choices.
  • Document patient encounters and outcomes in our electronic medical record system.
Requirements:
  • Graduate of an Advanced Nurse Practitioner program in Adult, Geriatric, or Family Medicine.
  • Active, unrestricted NP license in Pennsylvania.
  • At least 1 year of experience in Primary Care, Geriatric, Adult, Home Care, or Family Medicine.
  • Current BLS certification; ACLS preferred.
  • Ability to be credentialed by Medicare, Medicaid, and other Private Insurance Companies.
  • DEA license and State Controlled Substance License upon hire or within 3 months of hire.
