Job Overview

This full-time position offers a unique opportunity to work in a dynamic healthcare setting, collaborating with multidisciplinary teams to deliver patient-centered care.

Key Responsibilities
  • Develop and implement evidence-based wellness and prevention initiatives
  • Participate in initial care plan development, including specific goals for all patients
  • Communicate effectively with medical providers, specialists, and other stakeholders regarding patient records and care plans
  • Provide education on health conditions, treatment options, medication management, and strategies to achieve care plan goals
  • Monitor assessments and screenings to ensure findings are integrated into the care plan
Facilitating Care Coordination
  • Utilize electronic health records (EHRs) and other healthcare information technology (HIT) to facilitate communication among team members and provide feedback
  • Monitor and report performance measures and outcomes
  • Collaborate regularly with the care team to plan care, discuss cases, and exchange information in an informal manner as part of the daily routine
Substance Abuse Treatment Expertise
  • Assess patients to ensure evidence of current physiological dependence, length of history of addiction, or exceptions to criteria for admission are documented in the patient record prior to receiving initial medications
  • Ensure that medical/mental health evaluations, including medical histories and physical examinations, have been performed prior to patients receiving initial medications
  • Conduct annual medical evaluations and 60-day medical evaluations on all patients prior to administering medications; make referrals as necessary
  • Order, perform, and review laboratory tests; sign-off on urinalysis results; review results with patients when necessary
  • Review medication lists and discuss drugs that should be tapered or discontinued due to interactions
Requirements and Qualifications
  • Must be fully licensed in the State of Michigan
  • Must conform to acceptable practices within the medical profession
  • Must be familiar with treating chemically dependent and mentally ill patients
  • Minimum of 1 year experience preferred
  • Hours of operation: Monday-Friday, 5:00 am-1:00 pm
  • Participate in ongoing internal and external training to maintain proficiency in current trends relative to providing quality medical care

The estimated salary range for this position is $55.00-$65.00 per hour, based on credentials and experience.
