Key Responsibilities:
- Provide occupational therapy services to patients in their homes, promoting independence and functional ability.
- Develop and implement personalized treatment plans, collaborating with patients, families, and healthcare teams.
- Conduct thorough assessments, identifying areas for improvement and developing strategies to address them.
- Communicate effectively with patients, families, and healthcare professionals, ensuring seamless care coordination.
- Stay up-to-date with industry developments, best practices, and evidence-based guidelines, applying this knowledge to enhance patient outcomes.
Requirements:
- Education: Degree from an accredited occupational therapy program.
- Licenses/Certifications:
- Valid license to practice occupational therapy in the state of practice.
- Current CPR/first responder certification.
- Valid Driver's License and Auto Insurance.
- Preferred qualifications: Home Health or Inpatient Rehab experience.
