Key Responsibilities:

  • Provide occupational therapy services to patients in their homes, promoting independence and functional ability.
  • Develop and implement personalized treatment plans, collaborating with patients, families, and healthcare teams.
  • Conduct thorough assessments, identifying areas for improvement and developing strategies to address them.
  • Communicate effectively with patients, families, and healthcare professionals, ensuring seamless care coordination.
  • Stay up-to-date with industry developments, best practices, and evidence-based guidelines, applying this knowledge to enhance patient outcomes.

Requirements:

  • Education: Degree from an accredited occupational therapy program.
  • Licenses/Certifications:
  • Valid license to practice occupational therapy in the state of practice.
  • Current CPR/first responder certification.
  • Valid Driver's License and Auto Insurance.
  • Preferred qualifications: Home Health or Inpatient Rehab experience.
