We are seeking a compassionate and experienced Family Consultant to join our team. In this role, you will provide direct and indirect social services to families caring for physically and cognitively impaired adults.

Responsibilities
  • Conduct comprehensive intakes and assessments of family caregivers' needs.
  • Provide information and referrals to community resources tailored to caregiver support.
  • Offer individualized family consultations to address specific challenges and concerns.
  • Develop and implement care management plans in collaboration with caregivers and healthcare professionals.
  • Facilitate support groups for caregivers, fostering a sense of community and shared experiences.
  • Conduct educational workshops on topics relevant to family caregivers, such as dementia care, stress management, and legal considerations.
Qualifications
  • Bachelor's degree in Social Work, Counseling, Gerontology, or a related field is required. A Master's degree or Licensed Therapist (or Associate) status is preferred.
  • Minimum of one year of experience providing social services to seniors, individuals with disabilities, healthcare settings, or mental health organizations.
  • In-depth understanding of the unique needs and challenges faced by family caregivers.
  • Extensive knowledge of local community resources available to support caregivers.
  • Excellent written and verbal communication skills.
  • Bilingual proficiency in English/Spanish is highly desirable.
  • Ability to work independently and manage multiple assignments concurrently.
  • Strong teamwork orientation and commitment to Del Oro Caregiver Resource Center's mission.
