Speech Language Pathologist

CenterWell Home Health is seeking a skilled Speech Language Pathologist to join our team. As a Speech Language Pathologist, you will work with patients in their homes to evaluate, direct, and provide speech/language pathology services. You will participate in the development and periodic review of the Plan of Treatment and Plan of Care, and utilize your professional skills and judgment to assess and treat disorders of speech, voice, language, hearing, and swallowing.
Key Responsibilities:
- Evaluate patients in their homes to assess their speech and language abilities.
- Develop and implement individualized treatment plans to address patients' speech and language needs.
- Provide speech and language therapy to patients in their homes.
- Participate in care coordination activities and discharge planning.
- Maintain accurate and detailed records of patient interactions and treatment plans.
Requirements:
- Master's degree in Speech-Language Pathology or related field.
- Current certification in Speech-Language Pathology (CCC-SLP) or eligible for certification.
- Minimum of six months experience as a Speech-Language Pathologist.
- Home health experience preferred.
- Current and unrestricted license to practice as a Speech-Language Pathologist.
- Current CPR certification.
- Good organizational and communication skills.
