Patient Care Coordinator for Oncology

We are seeking a highly skilled Patient Care Coordinator to join our team at Hunterdon Healthcare. As a key member of our interdisciplinary team, you will play a vital role in coordinating the overall patient care experience for individuals with known or suspected cancer diagnoses.
About the Role:
The Patient Care Coordinator will be responsible for facilitating a rapid and efficient transition through our system from referral to diagnosis and treatment. This involves collaborating with healthcare professionals to ensure seamless communication and coordination across various departments, including medical oncology, radiation, surgery, and hospital care. You will also provide essential patient education regarding their diagnosis and treatment plan, addressing any barriers to care and promoting self-advocacy. Additionally, you will serve as a liaison between patients, families, and healthcare providers, fostering strong relationships built on empathy, trust, and effective communication.
Your Key Responsibilities:
* Coordinate patient-centered care throughout the initial course of treatment, navigating multiple areas of care (medical oncology, radiation, surgery, hospital) to ensure continuity and optimal outcomes
* Advocate for patients and families, providing personalized education and support to empower informed decision-making and address potential barriers to care
* Utilize exceptional organizational and interprofessional collaboration skills to manage resources effectively and facilitate smooth transitions within the healthcare system
* Participate in quality improvement initiatives and contribute expertise to activities that strengthen our cancer program, encompassing data collection, analysis, continuous education, and protocol implementation
* Foster a culture of lifelong learning by actively seeking and engaging in educational opportunities to enhance knowledge and skills relevant to job responsibilities
Requirements:
We are seeking candidates with a minimum of three years of experience as a registered nurse, preferably with a bachelor's degree in nursing. Your background should include recent acute care experience, with a strong preference for oncology-specific experience. We require NJ state licensure and American Heart Association CPR certification. Additionally, certification in oncology is desired, such as an Oncology Certified Nurse or equivalent. A demonstrated commitment to continuous quality improvement, lifelong learning, and excellent interpersonal skills are essential for this role. Strong computer skills, including proficiency in Google Workspace, are also necessary.
