Job Summary

Accentra Home Healthcare is seeking a compassionate and skilled Hospice Registered Nurse (RN) to provide high-quality end-of-life care to patients in their homes. As a Hospice RN, you will work closely with the interdisciplinary team to develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families.

Key Responsibilities:

  • Deliver high-quality, patient-centered care to patients in their homes, including routine visits, skilled nursing, and supervisory visits.
  • Develop and implement individualized care plans that address the physical, emotional, and spiritual needs of patients and their families.
  • Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
  • Provide education and support to patients and their families on disease management, symptom control, and end-of-life care.
  • Document patient care and progress in a timely and accurate manner.
  • Participate in case conferences and care coordination meetings with healthcare partners to ensure avoidance of duplication of services and coordinated care.

Requirements:

  • Current licensure as a Registered Nurse in the State of Oklahoma.
  • At least 2 years of hospice experience as an RN preferred.
  • Certification in Hospice and Palliative Nursing a huge plus.
  • Excellent verbal and written communication skills.
  • Ability to maintain concentration during frequent interruptions.
  • Valid driver's license, reliable vehicle, and valid auto liability insurance.
