Position Summary:

  • The pharmacist will play a vital role in our expanding hematology and oncology practice by dispensing medications prescribed by our healthcare providers, formulating treatment regimens, and collaborating closely with the Clinical Pharmacist Manager and pharmacy technicians to ensure the safe handling and delivery of medications to our patients.

Key Responsibilities:

  • Collaborate as part of a multidisciplinary team including oncologists, advanced practice providers, and nursing staff.
  • Assess and verify chemotherapy orders through comprehensive profile and laboratory evaluations.
  • Act as a medication specialist by providing drug information and promoting the optimal use of pharmaceuticals.
  • Participate in various multidisciplinary committees representing the pharmacy department.
  • Engage in formulary management and the development of medication use policies for oncology-specific drugs.
  • Review and monitor patient treatment plans daily, ensuring accurate dosing and timing, adjusting as necessary based on lab results, and providing recommendations for side effects management.
  • Offer guidance on drug interactions, side effects, dosages, and proper medication storage practices.
  • Develop and maintain procedures for the preparation, packaging, and labeling of pharmaceuticals in compliance with legal and policy standards to ensure quality and safety.
  • Work alongside other healthcare professionals to evaluate the effectiveness of drug regimens and provide expert advice on drug applications.
  • Create and assess regimen templates for the practice, ensuring accuracy in premedications, administration duration, sequencing, and laboratory requirements.
  • Analyze prescribing patterns to monitor patient adherence and prevent adverse drug interactions.
  • Manage the ordering and procurement of pharmaceutical and medical supplies, ensuring proper storage and handling.
  • Assist with prior authorization processes for treatments.
  • Educate staff on new medications, including reconstitution, administration techniques, side effects, and management strategies.
  • Provide consultation on safe handling of hazardous drugs and other pharmacy-related inquiries.
  • Assume supervisory responsibilities in the absence of the Clinical Pharmacist Manager.

Required Skills and Qualifications:

  • Demonstrated compassion and a caring approach to patient interactions.
  • Strong knowledge of chemistry and drug interactions.
  • Exceptional attention to detail with the ability to manage multiple priorities effectively.

Preferred Education and Experience:

  • Doctor of Pharmacy (Pharm.D.) degree.
  • Completion of a PGY-2 residency in oncology pharmacy or Board Certified Oncology Pharmacist (BCOP) certification preferred.
  • Active pharmacy license required.

Physical Requirements:

  • Ability to sit for extended periods while working at a desk and using a computer.
  • Capability to lift up to 15 pounds occasionally.
