Job Title: Registered Nurse Case Manager

Job Summary:

We are seeking a skilled Registered Nurse Case Manager to join our team at Interim HealthCare of New Hampshire. As a Registered Nurse Case Manager, you will be responsible for managing a defined caseload of patients, providing direct patient care and comprehensive nursing services to individuals in the community.

Responsibilities:

  • Manage a defined caseload of patients
  • Provide direct patient care and comprehensive nursing services
  • Collaborate with healthcare teams to develop and implement patient care plans
  • Conduct regular patient assessments and evaluations
  • Develop and implement individualized patient care plans

Requirements:

  • Valid nursing license in the state of NH
  • 2+ years of experience in skilled care preferred
  • 1+ year of experience in nursing supervision preferred
  • Recent wound care experience
  • Must be able to lift 50 LBS with no restrictions
  • Current CPR certification
  • Reliable transportation
