Job Summary

Key Responsibilities
  • Assess and develop discharge plans for adult and geriatric patients, taking into account their unique needs and circumstances.
  • Collaborate with patients, families, physicians, and other healthcare professionals to ensure comprehensive care and support.
  • Identify and address emotional, social, and financial challenges faced by patients and families, providing guidance and resources as needed.
  • Work with patients and families to develop coping strategies and provide emotional support during times of crisis.
  • Develop and implement care plans that address the physical, emotional, and social needs of patients and families.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination.
  • Maintain accurate and timely documentation of patient interactions and care plans.
Requirements
  • Master's Degree in Social Work or a related field from an accredited institution.
  • Current licensure in the State of Maryland - LMSW/LGPC required; LCSW-C/LCPC preferred.
  • Minimum of 1 year of experience in a healthcare setting, preferably in a hospital or social services role.
