Job Summary

We are seeking a skilled and compassionate RN Case Manager to join our team at El Paso Children's Hospital. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality patient care and outcomes.

Key Responsibilities
  • Comprehensively plan and coordinate case management services for a targeted patient population, utilizing evidence-based clinical pathways and best practices.
  • Collaborate with physicians, social services, and interdisciplinary teams to facilitate clinical pathways and achieve desired treatment outcomes.
  • Coordinate and oversee the care and service of patients to achieve desired clinical and resource outcomes, ensuring seamless transitions between care settings.
  • Provide education and support to patients and families related to their medical condition, promoting health literacy and self-management skills.
  • Assess patient needs for continued care, applying utilization review concepts according to federal, state, and JCAHO guidelines.
Requirements
  • Clinical expertise and strong analytical, data management, and computer skills.
  • Excellent interpersonal communication and negotiation skills, with the ability to work effectively with diverse populations.
  • Strong organizational and time management skills, with the capacity to prioritize multiple tasks and role components.
  • Ability to work independently and exercise sound judgment in interactions with physicians, payors, and patients/families.
  • Bilingual English/Spanish preferred, with cultural sensitivity and awareness.
  • Current Texas RN License and CPR Certification required.
  • Associates degree in Nursing required, with Bachelors of Science in Nursing preferred.
Preferred Qualifications
  • Previous case management experience, with a focus on pediatric care.
  • Certified Case Manager (CCM) certification.
