About the Role

The successful candidate will be responsible for:

  • Coordinating and managing patient care in a long-term care setting, ensuring seamless transitions between acute and long-term care settings.
  • Performing utilization review to optimize resource allocation and ensure efficient use of resources.
  • Developing and implementing comprehensive discharge plans that meet patients' unique needs and goals.
  • Collaborating with healthcare professionals to deliver coordinated and person-centered care.
  • Maintaining accurate and detailed patient records, documenting all aspects of patient care and treatment.
  • Providing leadership and guidance to nursing staff involved in patient care coordination, promoting best practices and continuous improvement.
  • Communicating effectively with patients, families, and healthcare teams, including those who require language support.
Requirements

To be considered for this role, you must have:

  • A Bachelor's degree in Nursing (BSN) or higher.
  • At least five years of clinical experience as a Registered Nurse in a certified home health agency, Lombardi program, or managed long-term care or acute program.
  • Leadership experience and a proven track record of success in care coordination, utilization review, and discharge planning.
  • PRI and Screen certification required within three months of employment; Case Management certification preferred.
  • Bilingualism in Spanish (read, write, speak) is highly desirable.
