Responsibilities- Assessment and Planning: Conduct thorough assessments of patients to identify their physical, emotional, and social needs.
- Develop and Implement Care Plans: Create and implement individualized care plans that address patient needs and promote optimal health outcomes.
- Provide Direct Patient Care: Administer medications, perform various medical procedures, and provide education to patients and their families.
- Collaborate with Interdisciplinary Teams: Work closely with physicians, nurses, and other healthcare professionals to ensure seamless care coordination and communication.
- Participate in Quality Improvement Initiatives: Contribute to quality improvement efforts to enhance patient satisfaction, safety, and outcomes.
