We are seeking a skilled Physical Therapist to join our team at Medical Center Home Health, a part of LHC Group. As a Physical Therapist, you will play a vital role in helping patients recover from injuries or illnesses and regain their independence.

About the Role:
  • Assess and evaluate patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions.
  • Develop and implement treatment plans to improve and enhance patient well-being.
  • Evaluate patient progress and adjust treatment plans as needed.
  • Collaborate with healthcare teams to ensure comprehensive care.
Responsibilities:
  • Evaluate patient functional status and physical therapy needs.
  • Assist physicians in developing therapy plans of care.
  • Observe and report patient responses to treatment and changes in condition to supervising nurses and physicians.
  • Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using acceptable standards of practice.
  • Instruct and inform participating members of the healthcare team, patients, and family/caregivers regarding the plan of care, functional limitations, and progress toward goals.
Requirements:
  • Current Physical Therapy licensure in the state of practice.
  • Current CPR certification required.
  • Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
