Job Summary

We are seeking a highly skilled Post Acute Transition Specialist to join our team at CarePartners Health Services. This role will act as a coordinator between internal and external referral sources and our Post-Acute Service Lines, ensuring seamless transitions for patients.

Key Responsibilities
  • Establish relationships with hospitals, skilled nursing facilities, and other community referral sources to promote collaboration and education.
  • Provide screenings and assessments in collaboration with SNF Discharge Planners & Inter-Disciplinary Teams, Ambulatory Care Managers, and other healthcare professionals to ensure appropriate patient placement.
  • Work closely with Post-Acute providers, SNFs, and physician practices to ensure a smooth referral and admission process for our Post-Acute programs.
  • Collaborate with health system and community colleagues to promote quality-driven, best practice across the continuum.
  • Maximize positive financial outcomes for designated patient populations by facilitating appropriate Post-Acute placement and participating in Goals of Care/Advanced Care Planning dialogue.
Requirements
  • Minimum of two years recent experience in a health-related field.
  • Required education: Bachelor of Science in Nursing, Degree in Physical, Occupational, or Speech Therapy.
  • Preferred education: Master's in Nursing or Master's degree in a related field.
  • Required license/certifications: If RN, must have and maintain current licensure as a registered nurse with the North Carolina Board of Nursing.
