About the Role:
  • Provide high-quality nursing care to patients in their homes, promoting their well-being and independence.
  • Assess patient needs, develop care plans, and implement interventions to achieve optimal health outcomes.
  • Collaborate with healthcare teams to ensure seamless care transitions and coordinate services.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making.
Requirements:
  • Current unrestricted RN license in the state of Texas.
  • Preferred: Bachelor's degree in Nursing.
  • Minimum one (1) year of documented experience in a home health agency, primary care clinic, or health facility.
  • Proven experience in wound care, IV skills, and central line care.
  • Certified CPR and negative TB skin test or chest x-ray.
