Job Summary

Responsibilities
  • Conduct comprehensive health assessments and develop individualized treatment plans
  • Provide high-quality patient care, including medication management, wound care, and education

  • Participate in quality improvement initiatives and contribute to a culture of excellence
  • Maintain accurate and up-to-date patient records, adhering to regulatory requirements
Requirements
  • Graduate of an accredited school of nursing and an accredited nurse practitioner program
  • Current and unrestricted state license as a Registered Nurse and required state authority to practice as a nurse practitioner
  • At least one year of experience as a nurse practitioner in a clinical setting
  • CPR certification and health and screening tests as required by client facilities and/or regulatory agencies
