Key Responsibilities- Provide nursing visits to patients in their homes, following treatment plans prescribed by physicians
- Conduct thorough assessments, develop, and implement individualized care plans to meet patients' needs
- Collaborate with healthcare teams to ensure seamless care coordination and communication
- Maintain accurate and complete medical records, adhering to regulatory requirements
- One year of clinical experience and one year of home health experience
- Current licensure as a Licensed Practical Nurse or Licensed Vocational Nurse in the state of agency operation
- Valid CPR certification
- Ability to travel to all business locations
- Competitive hourly rate: $22.50-$24.00
