Job Summary

We are seeking a compassionate and dedicated Caregiver to provide exceptional care to our clients in their home environment. As a Caregiver, you will be responsible for implementing and maintaining a professional and caring relationship with our clients, providing reliable care, and adhering to our Standard Operations Procedures and South Dakota State Regulations.

Key Responsibilities
  • Provide direct supervision and daily care of clients, primarily in their home environment.
  • Implement and maintain a professional and caring relationship with clients, providing reliable care, limit setting, and boundaries.
  • Plan and participate in activities with clients as outlined in the care plan.
  • Attend and actively participate in weekly team meetings, group supervision, and complete in-service hours annually as required by state regulations.
  • Maintain a professional, courteous, and helpful relationship with clients' family and other primary social supporters.
  • Adhere to Stay Graceful Standard Operations Procedures and South Dakota State Regulations/Administrative Rules relating to home care.
  • Assure all clients, coworkers, and family members are treated equally and with respect.
  • Responsible for the quality of services and completion of necessary home care duties and related training.
Requirements
  • No degree required; applicable experience strongly preferred in Human Services or a related field.
  • Excellent abilities in communication via email, Word, and medical records preferred.
  • Current South Dakota driver's license, proof of insurance, and clearance through the Division of Criminal Investigation required.
Physical Requirements
  • Ability to perform duties as outlined above, including lifting 10-20 lbs.
  • Continual standing and walking during shift.
  • Continual talking in person or on the phone during shift.

Please note that applicants who have been charged with a DUI within the past 5 years or have ever been charged with a violent crime or felony are ineligible for employment with Stay Graceful.
