LPN Supervisor Opportunity

We are seeking a compassionate and experienced LPN Supervisor to join our team at CommuniCare Family of Companies. As a key member of our care team, you will be responsible for providing exceptional patient care and leading a team of dedicated healthcare professionals.

Responsibilities:
  • Provide direct patient care and supervision to LPNs and CNAs
  • Develop and implement care plans that meet the unique needs of each patient
  • Collaborate with the interdisciplinary team to ensure seamless care transitions
  • Conduct regular staff evaluations and provide coaching and development opportunities
  • Ensure compliance with regulatory requirements and company policies
Requirements:
  • Registered Nurse licensure in the state of Indiana
  • Previous long-term care and supervisory experience
  • Strong leadership and communication skills
  • Ability to work in a fast-paced environment and prioritize tasks effectively
