Job Title: Physical Therapist - Skilled Nursing Facility

We are seeking a skilled and dedicated Physical Therapist to join our team at a skilled nursing facility in Live Oak, California. As a Physical Therapist, you will play a vital role in helping patients recover from physical ailments and regain their independence.

Job Summary:

The successful candidate will be responsible for evaluating patients, developing individualized plans of care, and providing therapy and rehabilitation services to promote effective and safe recovery. You will work collaboratively with our interdisciplinary team to ensure that patients receive comprehensive care and achieve their goals.

Responsibilities:
  • Evaluate patients to identify diagnosis and develop comprehensive plans of care
  • Provide therapy and rehabilitation services to promote effective and safe recovery
  • Work collaboratively with the interdisciplinary team to ensure comprehensive care
  • Document patient progress according to approved medical and facility documentation procedures
  • Discharge patients when goals have been met or medical necessity no longer warrants intervention
Requirements:
  • Active state license as a physical therapist
  • Current BLS/CPR certification
  • 6 months of PT experience within the last 5 years
