About the Role

We are seeking a skilled Occupational Therapist to join our team at CenterWell Home Health. As a key member of our care team, you will play a vital role in helping patients recover from illness, injury, or surgery and regain their independence.

Key Responsibilities
  • Assess patients' daily living and work-related skills, developing personalized therapeutic retraining programs with measurable objectives.
  • Administer and interpret diagnostic and prognostic tests to assist physicians in evaluating patients' level of function.
  • Collaborate with healthcare team members to develop and revise treatment plans, ensuring patients receive comprehensive care.
  • Provide therapeutic treatment and instruction to patients, improving strength, coordination, range-of-motion, and function, or teaching compensation measures.
  • Review and expand retraining programs to align with patients' behavioral goals and rehabilitation program guidelines.
  • Consult with vocational team members to develop vocationally oriented plans, addressing patients' needs and capabilities.
  • Communicate patient status and progress to physicians, rehabilitation staff, Clinical Manager, patients, and family members.
  • Participate in care coordination and discharge planning activities, serving as a resource for other healthcare personnel.
  • Design community reintegration activities to support patients' physical reconditioning and psychological adjustment, coordinating plans with interdisciplinary team members.
  • Recommend and design special adaptive equipment to enhance patients' residence or working environments or participation in the rehabilitation program.
  • Train staff and family members to carry out retraining programs, including dressing, feeding, grooming, and hygiene skills, participating in team and family educational meetings.
  • Supervise and evaluate Occupational Therapy Assistant and Home Health Aide performance in implementing occupational therapy services.
  • Accurately document patients' care observations, interventions, and evaluation, ensuring interim orders are accurately documented and implemented.
Requirements
  • Degree from an accredited Occupational Therapy Program.
  • A minimum of six months of occupational therapy experience, with Home Health experience preferred.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Good organizational and communication skills.
  • A valid driver's license, auto insurance, and reliable transportation.
