Job Summary

We are seeking a highly skilled and compassionate Mental Health Case Manager to join our Mobile Crisis Response Team. As a key member of our team, you will provide mobile mental health crisis and outreach services during business hours, Monday through Friday. Your primary goal will be to resolve emergency situations by providing brief and intensive services, reducing the need for hospitalization, diverting from justice system contact, providing crisis intervention, and offering assertive outreach.

Key Responsibilities
  • Engage individuals experiencing a behavioral health crisis, building rapport, assessing motivation, and encouraging participation in services.
  • Carry a crisis case management caseload, connecting individuals who have multi-faceted needs to ongoing supports and services.
  • Assess individuals' circumstances to identify needs and/or determine service eligibility.
  • Respond quickly in crisis moments to various community locations to provide assessment, intervention, referral, and connection to higher level of care.
  • Work closely with other Johnson County Mental Health Center (JCMHC) Emergency Services teams and law enforcement to coordinate care for individuals in crisis.
  • Network with other community agencies to coordinate resources and services for client benefit.
  • Complete and submit documentation regarding services provided.
Requirements
  • Bachelor's degree in Human Services or related field is required.
  • May require KS/MO driver's license, depending on department or agency.
  • Johnson County Government requires reference/background screening for all positions.
