About the Role

We are seeking a compassionate and skilled Certified Nursing Assistant to join our team at Grane Home Health & Hospice. As a Hospice Aide, you will play a vital role in providing personalized care to patients in their homes, supporting their physical, emotional, and spiritual needs.

Responsibilities
  • Provide personal care services, including bathing, dressing, and grooming, as assigned by the patient's care plan.
  • Assist with daily living activities, such as meal preparation, feeding, and ambulation, as needed.
  • Monitor and report any changes in the patient's condition to the Case Manager.
  • Collaborate with the hospice team to ensure seamless care coordination and communication.
  • Document patient care and progress accurately and thoroughly.
Requirements
  • Meet the training requirements of the State, including a minimum of 75 hours of classroom and supervised practical training.
  • Have a minimum of six months of experience in providing personal care services.
  • Be at least 18 years of age and possess a high school diploma or equivalent.
  • Have strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare professionals.
  • Be able to lift, push, and pull up to 25 pounds, with frequent standing, walking, and bending.
