Job Overview

We are currently seeking a skilled Nurse Practitioner to join our Institutional Special Needs Plan (ISNP) program in Medford, Oregon.

Estimated Salary

$120,000 - $150,000 per year, depending on experience.

Key Responsibilities

The successful candidate will be responsible for performing comprehensive clinical assessments on residents, developing individualized care plans, and providing primary, preventative, and urgent care. They will also collaborate with primary care physicians and facility staff, communicate with resident families, and assist with smooth transitions for patients entering the facility post-hospital discharge.

Requirements
  • Active Nurse Practitioner license in Oregon
  • Experience in geriatric or long-term care is preferred
  • Excellent communication and collaboration skills
