Job Summary

We are seeking a skilled and compassionate Home Health Registered Nurse (RN) to join our team at Compassus. As a Home Health Registered Nurse, you will play a vital role in providing patient-centered care and support to individuals in need of home health services.

Key Responsibilities

  • Provide high-quality patient care and support to individuals in their homes, including assessing patient needs, developing and implementing care plans, and providing education and training to patients and families.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive and coordinated care.
  • Conduct regular home visits to assess patient progress, provide ongoing care and support, and make adjustments to care plans as needed.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and transitions.
  • Participate in quality improvement initiatives and contribute to the development of policies and procedures to ensure high-quality patient care.

Requirements

  • RN license in the state of employment
  • Two years of experience as a registered nurse, with a preference for home health experience
  • Current driver's license
  • Current CPR and BCLS certification
