Job Description

We are seeking a dedicated Personalized Patient Care Specialist to join our team at FLOWERS HOME CARE, LLC. This role involves providing one-on-one care to patients in their homes, adhering to a personalized care plan.

About the Role:

  • Assist patients with daily activities such as mobility, personal hygiene, and meal preparation.
  • Care for patients by maintaining a clean and safe environment, including laundry and light housekeeping tasks.
  • Document patient care, condition, progress, and any concerns or issues to share with supervisors.
  • Engage patients through conversation, reading aloud, or other activities to promote mental health and well-being.
  • Administer medications and immunizations as prescribed.
  • Foster relationships with patients by actively listening and showing empathy.

Requirements:

  • Degree from an accredited Home Health Aide program.
  • High School Diploma or equivalent.
  • Minimum 1 year of professional experience in healthcare.
  • Valid driver's license and reliable transportation.
  • CPR certification and Certified Nurse Aid Certification.
