Job Title: Mental Health Case Manager

We are seeking a skilled professional to provide blended case management services for individuals with mental health needs. The successful candidate will promote recovery and well-being through a trauma-informed approach.

Main Responsibilities:
  • Provide Blended Case Management services under the direction of the Blended Case Management Supervisor.
  • Utilize strong customer service skills to engage individuals, families, and providers in goal identification and development.
  • Build relationships and lead teams to support individuals living their best lives.
  • Collaborate with community resources to link individuals to necessary services.
  • Assess needs and complete Individualized Service Plans.
  • Document case notes and be available to work on-call shifts.
Requirements:
  • Bachelor's Degree in psychology, social work, counseling, or related field.
  • Or, 12 credit hours in psychology, social work, counseling, or related field and 2 years experience.
  • MH/DD experience preferred.
  • Automobile and Valid Insurance required.
  • Valid Pennsylvania Driver's License required.
Working Conditions:

Prolonged periods working on a computer in an open office environment, comfortable communicating through virtual technology, work may be required at nights and on weekends, travel throughout the region, travel may be in adverse weather conditions, must be able to lift to 15 pounds.

Estimated Salary: $45,000 - $60,000 per year.
