Responsibilities:
  • Conduct thorough assessments of patients' physical and emotional needs

  • Collaborate with healthcare professionals to ensure seamless care transitions and coordination

Requirements:
  • Current Registered Nurse (RN) license in the state of Illinois
  • One year of prior RN experience, preferably in home healthcare or a related field
  • Strong communication and interpersonal skills

  • Basic Life Support (BLS) certification
