Job SummaryWe are seeking a skilled and compassionate Physical Therapist to join our team of healthcare professionals. As a Physical Therapist with Elara Caring, you will have the opportunity to work with patients in their homes, providing personalized care and support to help them achieve their goals and improve their quality of life.
Responsibilities- Conduct thorough assessments of patients' physical abilities and develop personalized treatment plans to address their needs and goals.
- Provide one-on-one physical therapy sessions to patients in their homes, using a variety of techniques and modalities to promote healing and recovery.
- Collaborate with other members of the healthcare team, including physicians, nurses, and social workers, to ensure that patients receive comprehensive and coordinated care.
- Communicate effectively with patients and their families, providing education and support to help them understand their treatment plans and make informed decisions about their care.
- Document patient progress and outcomes, using data to inform treatment decisions and improve patient outcomes.
- Current Physical Therapy license in the state of Michigan.
- 1 year of experience in a healthcare setting, preferably in home health or geriatric care.
- Ability to meet the physical demands of the job, including lifting, bending, and standing for long periods.
