Coordinated Entry Program Specialist

As a key member of our team, the Coordinated Entry Program Specialist will support the CES Program Manager in the implementation and on-going administration, development, and continuous improvement of Contra Costa's coordinated entry system.

The Program Specialist is responsible for supporting care coordination of CE providers in Contra Costa, acting as the role of ombudsman for CE complaints and grievances, meeting facilitation and general CE support. Key responsibilities include:

  • Supporting the CES Program Manager in identifying performance goals for CE and working with program staff and CE providers to ensure that all internal and external performance goals are being met.
  • Facilitating or co-facilitating regularly scheduled Housing Assessment and Placement (HAP) case conferences as units become available to review and place prioritized clients, assign new individuals to housing navigators, review and monitor progress of cases already assigned.
  • Supporting CES Program Manager in maintaining the centralized housing placement list.
  • Responding to community requests for information and facilitating trainings/presentations as needed.

The ideal candidate will have a Master's degree in Social Work, Public Health, Public Administration, Counseling, or a closely related field, and a minimum of two years of full-time experience or its equivalent providing direct service delivery in housing and/or homeless services, and a minimum of 1 year of full-time experience or its equivalent coordinating and developing housing/homeless services in an administrative capacity.
