VNS Health is seeking a skilled Physical Therapist to join our team in providing high-quality, patient-centered care in the comfort of patients' homes and communities.
Key Responsibilities- Conduct comprehensive assessments of patients to determine their needs and develop personalized treatment plans.
- Deliver rehabilitation therapy to patients in their homes or care facilities, focusing on their unique needs and goals.
- Collaborate with interdisciplinary care teams to ensure seamless care coordination and patient outcomes.
- Continuously evaluate patient needs and adjust treatment plans accordingly.
- Minimum of one year of physical therapy experience or successful completion of probationary period as a VNS Health Rehab Intern.
- Licensure to practice as a Physical Therapist in New York State.
- Valid driver's license required for all areas outside the borough of Manhattan.
- Competitive pay rates, with a hiring range of $70-$75 per visit for areas outside the Bronx and $90-$95 per visit for the Bronx.
