Key Responsibilities:- Provide personalized support and care to individuals with complex needs
- Develop and implement personalized plans to promote independence and community involvement
- Collaborate with a multidisciplinary team to ensure comprehensive care and support
- Assist with daily living tasks, including personal hygiene and medication management
