REPORTING STRUCTURE:
Branch Supervisor

ESSENTIAL QUALIFICATIONS

Must fulfill the educational and experiential criteria for a Certificate of Clinical Competence in Speech-Language Pathology or Audiology, as awarded by the American Speech-Language-Hearing Association (ASHA). Must possess the necessary certification or licensure to practice Speech-Language Pathology in the state.

A minimum of one year of clinical experience in Speech-Language Pathology is preferred.
Strong oral and written communication skills are essential, along with effective organizational abilities.
Demonstrates proficiency in communication techniques suitable for various age groups, with a focus on the geriatric demographic.

OVERVIEW OF JOB FUNCTIONS

The qualified therapist collaborates with the physician to assess functional levels, assists in formulating the care plan (modifying it as required), prepares clinical documentation and progress reports, and provides guidance and consultation to families and other agency staff; actively participates in in-service training programs. The speech-language pathologist is accountable for upholding the standards of care for speech-language pathology services.


KEY RESPONSIBILITIES

Delivers speech pathology or audiology services in alignment with the care plan authorized by the physician.

Accepts referrals exclusively from the Branch Supervisor.

Assists the physician in evaluating the patient/client's functional status and the formulation (and revision, as necessary) of the care plan by employing appropriate assessments for speech, hearing, and language disorders.

Evaluates the home setting (when applicable), identifies necessary equipment related to speech/language functionality.
Assesses the patient/client's communication abilities, oral-motor function, cognitive capacity, memory, problem-solving skills, hearing, and speech fluency and quality. Identifies functional speech impairments and develops a care plan aimed at enhancing the patient/client's functionality.
Implements speech therapy programs utilizing specialized therapeutic techniques and/or equipment.
Recommends communication devices/aids as needed. Suggests hearing evaluations by an audiologist. Checks hearing aids.

Documents and communicates to the physician and/or Branch Supervisor and RN Case Manager the patient/client's response to treatment and any alterations in the patient/client's condition.

Collaborates with the RN Case Manager to ensure cohesive and comprehensive care that contributes to the overall care plan.
Develops, prepares, and maintains individualized patient/client care progress records with precision, punctuality, and in accordance with policies. Submits accurate documentation.
Maintains clinical expertise in speech-language pathology practices and theories. Responsible for staying updated on trends and knowledge in speech pathology. Engages in in-service training programs.
Coordinates the comprehensive care plan and ensures continuity of patient/client care by liaising with other healthcare professionals. Participates in patient/client care coordination. Initiates patient/client care coordination for complex and/or multidisciplinary cases whenever necessary to ensure effective care coordination. Identifies patient/client needs for additional services and refers as appropriate. Plans for patient/client discharge from services.

Instructs and advises patients/clients and/or family members/caregivers and other healthcare personnel on aspects of speech pathology relevant to the patient's/client's treatment plan as indicated.

Supervises home health aides at least every 14 days, instructing them in duties and procedures directly related to the speech-language therapy care plan as needed, assesses the patients/clients' need for unskilled care, and revises the care plan as necessary.

Communicates changes in the Plan of Care to the patient/client and Branch Supervisor/RN Case Manager as often as necessary. Communicates Plan of Care changes to the Aide as appropriate.
Performs additional duties as required to facilitate the delivery of speech-language pathology services.
Collaborates and consults with relevant staff to provide staff education when requested and arranged by the Branch Supervisor.
Responsible for adhering to all agency policies and procedures regarding service delivery, documentation, and care coordination.
Participates in the agency's Quality Improvement program. Can articulate own responsibilities and activities.

This position is classified as Class I Field clinical staff (refer to Infection/Exposure Control Plan).

Employees in Class I Field clinical staff roles may encounter potential exposure to bloodborne pathogens and other potentially infectious materials.

All Class I Field clinical staff employees will be offered Hepatitis B vaccination at no cost to the employee.

Must fulfill the required continuing education hours for state licensure as applicable.

Agency personnel are expected to engage in appropriate continuing education as may be requested and/or mandated by their immediate supervisor.

Additionally, agency personnel are expected to take personal responsibility for other educational activities to enhance job-related skills and competencies.

All agency personnel must attend mandatory educational programs.
